Comedone extraction The removal of open or closed comedones by applying light pressure over individual lesions with a comedo extractor may be a useful adjunct to more conventional acne therapy. In theory, the removal of closed comedones may prevent the eventual formation of inflammatory lesions.

Skill and patience is required to produce positive results. Patients should be properly motivated because of the moderate discomfort and the temporary detraction to their appearance. The eccentrically placed punctum of a whitehead can be seen under a good light and the orifice enlarged, gently nicking the overlying epidermis with a small blade or needle.

Pressure is then exerted over the lesion using various shapes of comedo extractor..

Removal of blackheads needs no explanation. However, overzealous attempts at comedo removal may result in iatrogenic papules.
